Somnath Temple, one of the 12 Jyotirlingas of Lord Shiva, is located in Prabhas Patan near Veraval in Gujarat. It has been destroyed and rebuilt multiple times throughout history, most recently in 1951. Punjab, Orissa, and Goa are incorrect as they house different famous temples.
