Maternal Health: Uterine Rupture

The most common cause of uterine rupture is previous cesarean section, followed by prolonged labor and macrosomia. Other risk factors include uterine anomalies, multiple gestations, and placenta previa.

The signs and symptoms of uterine rupture include abdominal pain, vaginal bleeding, fetal distress, and shock. The pain is typically severe and sudden, and may be accompanied by a tearing or ripping sensation. Vaginal bleeding may be heavy or light, and may contain clots. Fetal distress may be indicated by a change in the fetal heart rate or a decrease in fetal movement.

The treatment for uterine rupture is immediate cesarean section, blood transfusion, and repair of the uterine rupture. The cesarean section is performed to deliver the baby and placenta, and to control the bleeding. The blood transfusion is given to replace the blood that has been lost. The uterine rupture is repaired surgically.

The risks of uterine rupture include maternal death, fetal death, permanent disability, and future infertility. Maternal death is the most serious risk, and occurs in up to 10% of cases. Fetal death occurs in up to 50% of cases. Permanent disability can occur in up to 25% of cases, and may include paralysis, incontinence, and chronic pain. Future infertility is a risk in up to 10% of cases.

The best way to prevent uterine rupture is to avoid cesarean section, prolonged labor, and macrosomia. This can be done by having a vaginal delivery whenever possible, limiting the use of oxytocin to induce or augment labor, and managing gestational diabetes to prevent macrosomia.

Multiparity is not a risk factor for uterine rupture. In fact, it is associated with a decreased risk of uterine rupture.

The incidence of uterine rupture is approximately 1 in 2000 deliveries.
