"Does" is used with singular and before a verb in questions and negative statements. Hence, "doesn't" is correct answer.

In continuous tenses (1st form of verb + ing) either "was" or "were" is used. As it (she) is singular so "was" is correct option.

As it is present indefinite tense (first form of verb-like)  either "do" or "does" is used. And with singular (dog) "does" is used. Hence, does is the correct answer.
