In Inception's tragic irony, Cobb had earlier implanted an idea in Mal's mind during their limbo exile: that their world wasn't real and death was necessary to wake up. This suggestion, originally intended to help them escape limbo, took permanent root in Mal's subconscious. Even after returning to reality, she remained convinced that the waking world was itself a dream and that suicide was the only way to truly awaken.
