What invention caused many deaths while testing it?

Early parachute testing was extremely dangerous, with many inventors and testers dying during development. Fauste Veranzio and others tested prototypes by jumping from towers, and early designs were often fatal. Dynamite, ladders, and race cars can be dangerous but were not defined by causing deaths specifically during their testing phase.
